Dear Howard: Because you have so long been my friend and physician, I am afraid that sometimes I overlook telling you, as best I can, the true measure of my gratitude to you.1 I have, unwittingly to be sure, imposed a responsibility on you twice within the last year that would have staggered a lesser man; but you have responded brilliantly, with all the great skill and warm compassion at your command.2 All I can say is a simple "thank you"--but you will, with your understanding and devotion, know what those two words really mean.
